No matter what care your patients need, VNA Care can help. Across our home health care, palliative care, and hospice programs, VNA Care has the range of services to treat your patients at home, in nursing homes, through community programs, and at in-patient facilities.
 
VNA Care's Referral Center has extended hours to serve you better. Referral Center Representatives are available Monday through Friday 8 am to 6 pm and Saturday, Sunday, and holidays from 8 am to 4:30 pm. Referral Center staff can answer questions, arrange services, and clarify insurance coverage. A liaison nurse may be assigned to work directly with a patient's hospital, rehabilitation facility, nursing home, case manager, or physician to smooth the transition home.
 

Online Referrals

Online referrals may be made using our secure, online referral form. New users may email providerlinkrequests@vnacare.org for access. Learn more about how to make a referral through Provider Link in this video tutorial.
 

Fax Referrals

Physicians and others in the health care community may refer patients via fax. Faxes may be sent to 774-778-1007. For Medicare referrals, a Face-to-Face Encounter is required.
 
Please call our Referral Center at 800-728-1862 to confirm receipt of fax. Referral Center staff will contact the referral source within one hour of receiving the fax if additional information is needed or arrangements need to be discussed.

 

Physician Order Line

For any clinical questions, or home health plan of care changes for your active patients, please call our Physician Order Line at 508-751-6906 and speak directly with a VNA nurse. The Physician Order Line is available from 8 am to 4:30 pm Monday through Friday. Orders may also be reviewed and approved online (watch our tutorial). New users may email us to request access.
