Why VNA Care? VNA Care understands the challenges many face when wanting to remain safe and independent at home. We have been helping patients do just that for over 130 years. Our focus on clinical excellence and commitment to quality ensures our patients are receiving the best possible care where they prefer it most – in the comfort of their own home. PIONEER IN THE FIELD OF HOME HEALTH CARE, HOSPICE, AND COMMUNITY CARE VNA of Boston, a division of VNA Care, founded the first organized VNA in the United States in 1886. VNA Care Network, another division of VNA Care, traces its roots to 1891, when residents of Dedham raised money to start the Dedham Society for Emergency Nursing, later known as Visiting Nurse Associates. VNA Hospice Care opened their doors in 1979 as one of the first hospice providers in Massachusetts. We opened the first hospice home in Massachusetts in 1991 and currently operate three hospice residences in the state. Our organization includes the former Hospice of Central Massachusetts, which became the first Medicare certified hospice in the state in 1984. DEPTH AND BREADTH OF EXPERIENCE We offer more than 70 programs and services for a truly comprehensive, flexible approach to care. The range of care services offered means there's no need for a patient to be discharged from one health care provider and re-admitted to another as medical needs change. We are among only a few home health agencies able to provide a full complement of IV therapy services, extensive array of disease management and specialty services including diabetes, wound care, falls prevention, geriatric care, joint replacement therapies, and medication management. We also offer no cost health care services, such as Keep Well Clinics and fall prevention programs for seniors, promoting the health of local communities. EMPHASIS ON PATIENT AND FAMILY SATISFACTION Our nonprofit mission is centered on providing exceptional home care services to every patient we serve. Electronic patient record management system gives a complete picture of a patient's history day or night resulting in well-coordinated care. VNA Care’s knowledgeable staff connects patients and families to vital community resources such as Meals on Wheels, transportation, and support groups helping patients remain at home. Liaison nurses are available to help individuals and families transitioning from a medical facility to home. Our new innovative programs such as, Telemonitoring enhances care by offering "virtual" nursing visits in addition to traditional, in-person care. Use of this equipment has resulted in the early detection of and intervention with, health problems which decreases emergency room visits and hospital stays.
